A Form 11 (Request for Second Medical Opinion), is a form completed under the British Columbia Mental Health Act when a patient wants a second opinion on the appropriateness of their medical treatment.
The facility must provide the physician providing the second opinion with access to the patient and their records.
If the physician providing the second opinion recommends a different treatment plan, then the director/authorized physician will review the recommendation and will choose whether or not to authorize changes to the current treatment plan (but there is no obligation to do so).
