Form 11 (British Columbia - Request for Second Medical Opinion)

A Form 11 (Request for Second Medical Opinion), is a form completed under the British Columbia Mental Health Act when a patient wants a second opinion on the appropriateness of their medical treatment. The second opinion physician will then complete a Form 12 (Medical Report (Second Medical Opinion)).

Tips
- The facility must provide the physician providing the second opinion with access to the patient and their records.
- If the physician providing the second opinion recommends a different treatment plan, then the director/authorized physician will review the recommendation and will choose whether or not to authorize changes to the current treatment plan (but there is no obligation to do so).
